Patient Information

Dr. Chamblee’s appointment policy

Please call at least 24h in advance if you will be unable to keep your appointment. If you do not show up for your appointment without giving notice, there may be a delay of 4 months or more before another appointment is available. If you no-show more than once without notice, you will no longer be eligible to see Dr. Chamblee.

Medical insurance versus vision Insurance

Eye care is covered by two different types of insurance, medical insurance and vision insurance. Medical insurance is the same one that you use to go see your regular doctor and every other specialist. Vision insurance is designed to offset the cost of glasses or contact lenses, in addition to eye exams for people with no problems.

Refraction Policy

In general, medical insurances do not cover refractions, or the measurement of your eyes for a glasses prescription. We provide this service for a separate charge if you wish to have it performed at the same time as a medical eye exam. Vision insurances do generally cover the charge for refraction.

New Patient Policy

We are accepting new patients who wish to have an exam using their medical insurance or who are cash pay. We are not accepting new patients who wish to use their vision insurance. Please call if you are not sure what kind of insurance you have.

Vision Insurance is accepted by our optical shop

You may purchase glasses using your vision insurance in our Optical Shop regardless of what insurance you use for your exam. In addition, our optical shop does accept glasses prescriptions from outside ophthalmologists and optometrists.
